3.本項(xiàng)目的特定資格要求：

合同包1(陜西省人民醫(yī)院全院放射片袋采購(gòu)項(xiàng)目)特定資格要求如下:

（1）具有獨(dú)立承擔(dān)民事責(zé)任能力的法人、其他組織或自然人，并出具合法有效的營(yíng)業(yè)執(zhí)照或事業(yè)單位法人證書(shū)等國(guó)家規(guī)定的相關(guān)證明，自然人參與的提供其身份證明。?（2）財(cái)務(wù)狀況報(bào)告：提供2025年度經(jīng)審計(jì)的財(cái)務(wù)報(bào)告（包括“四表一注”，即資產(chǎn)負(fù)債表、利潤(rùn)表、現(xiàn)金流量表、所有者權(quán)益變動(dòng)表及其附注，成立時(shí)間至提交響應(yīng)文件截止時(shí)間不足一年的可提供成立后任意時(shí)段的資產(chǎn)負(fù)債表），或其談判前六個(gè)月內(nèi)銀行出具的資信證明。（以上兩種形式的資料提供任何一種即可）（3）稅收繳納證明：提供2025年8月至今已繳納至少一個(gè)月的依法繳納稅款的相關(guān)憑據(jù)（時(shí)間以稅款所屬日期為準(zhǔn)），憑據(jù)應(yīng)有稅務(wù)機(jī)關(guān)或代收機(jī)關(guān)的公章或業(yè)務(wù)專(zhuān)用章。依法免稅或無(wú)須繳納稅款的供應(yīng)商，應(yīng)提供相關(guān)證明文件。?（4）社會(huì)保障資金繳存證明：提供2025年8月至今已繳存的至少一個(gè)月的社會(huì)保障資金繳存單據(jù)或社保機(jī)構(gòu)開(kāi)具的社會(huì)保險(xiǎn)參保繳費(fèi)情況證明，依法不需要繳納社會(huì)保障資金的單位應(yīng)提供相關(guān)證明材料。?（5）書(shū)面聲明：參加本次采購(gòu)活動(dòng)前三年內(nèi)在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄，以及未被列入失信被執(zhí)行人、重大稅收違法失信主體、政府采購(gòu)嚴(yán)重違法失信行為記錄名單的書(shū)面聲明。本項(xiàng)目拒絕被列入失信被執(zhí)行人、重大稅收違法失信主體、政府采購(gòu)嚴(yán)重違法失信行為的供應(yīng)商參與。?（6）提供具有履行合同所必需的設(shè)備和專(zhuān)業(yè)技術(shù)能力的承諾函。?（7）法定代表人授權(quán)書(shū)及被授權(quán)人身份證。（法定代表人直接談判只須提交其身份證明）（8）本項(xiàng)目專(zhuān)門(mén)面向中小企業(yè)采購(gòu)，供應(yīng)商須為中小企業(yè)，并提供中小企業(yè)聲明函。
